2.8 Step Eight -The Virtual Visit (applies to Store/forward only)
The critical factor in a Store/forward visit is the accurate gathering
and transmission of the information needed by the consulting physician.
The patient will not be with the referring physician when the consultant
reviews the case. The gathering of additional information would likely
require another office visit by the patient.
Store/Forward Referring Provider Checklist:
1. Determine precisely what information will be needed by the consultant.
Examples: Photos (What views are required? Close-ups? Other?); Medical
Record (electronic or paper-based); Lab results.
2. Decide the method of transmission.
Examples: Email attachments for photos; Fax for paper medical records;
Online access of lab results.
3. Gather appropriate equipment: digital camera, memory device, tripod,
lighting, etc.
4. See patient: shoot photos, perform tests, gather any additional information
needed.
5. Verify quality of information gathered (before patient leaves).
Example: Check for sharpness of focus and accurate color in photos.
6. Transfer information to "sending" system.
Example: transfer digital photos to computer for transmission via email.
7. Send information to consultant: send email; fax records, etc.
8. Confirm receipt of transmission (ask for confirming email or telephone
call).
